Until January 1, 2019, the Affordable Care Act required individuals to have health insurance coverage, or qualify for an exemption, to avoid a tax payment. As part of this requirement, Blue Cross & Blue Shield of Mississippi was required by the Affordable Care Act to provide you each year with a Form 1095-B, which provided information about your Blue Cross & Blue Shield of Mississippi coverage for the year and whether you had qualifying health insurance coverage for tax filing purposes.
Beginning with the 2019 tax year, the Affordable Care Act no longer imposes a tax penalty for failure to have qualifying health coverage. Therefore, you do not need a 2019 Form 1095-B to complete your 2019 federal tax filing.
As in prior years, your electronic 2019 Form 1095-B is available now for download in the "My Info" section of our myBlue member portal, which is accessible by logging in above. Select the "My Info" tab after logging in and look for the "Form 1095-B" section. You can print your Form 1095-B from the My Info section at any time.
You can also request that a hard copy of your 2019 Form 1095-B be mailed to you in one of the following ways:
- By sending us a letter requesting your 2019 Form 1095-B. Please include your full name and your Blue Cross & Blue Shield of Mississippi ID number. The letter should be mailed to the following address: Blue Cross & Blue Shield of Mississippi Attn: 1095-B Request 3545 Lakeland Drive Flowood, MS 39232
- Contact our Customer Service Team at 601-664-4590 or 800-942-0278, 8:00 a.m. to 4:30 p.m. Monday through Friday.
If you have a Blue Cross & Blue Shield of Mississippi Medicare Supplement plan, you will need to contact Medicare directly (1-800-Medicare) for information on your 2019 Form 1095-B.
