Magnetic resonance imaging (MRI) is a multiplanar imaging method based on an interaction between radiofrequency (RF) electromagnetic fields and certain nuclei in the body (usually hydrogen nuclei) after the body has been placed in a strong magnetic field. MRI differentiates between normal and abnormal tissues, providing a sensitive examination to detect disease. This sensitivity is based on the high degree of inherent contrast due to variations in the magnetic relaxation properties of different tissues, both normal and diseased, and the dependence of the MRI signal on these tissue properties.
MRI is a radiology technique that uses magnetism, radio waves, and a computer to produce images of body structures. The MRI scanner is a tube surrounded by a giant circular magnet. The patient is placed on a moveable bed that is inserted into the magnet. The magnet creates a strong magnetic field that aligns the protons of hydrogen atoms, which are then exposed to a beam of radio waves. This spins the various protons of the body, and they produce a faint signal that is detected by the receiver portion of the MRI scanner. The receiver information is processed by a computer, and an image is produced. The image and resolution produced by MRI is quite detailed and can detect tiny changes of structures within the body. For some procedures, contrast agents, such as gadolinium, are used to increase the accuracy of the images.
MRI is a proven imaging modality for the detection, evaluation, assessment, staging, and follow-up of disorders of the upper extremity. Properly performed and interpreted, MRI not only contributes to diagnosis but also can guide treatment planning, help predict outcome, and increase diagnostic confidence. However, MRI should be performed only for a valid medical reason, and only after careful consideration of alternative imaging modalities. The strengths of MRI and other modalities should be weighed as to their suitability in particular patients and in particular clinical conditions.

Provider Accreditation for MRI – Network Providers
All Network Providers billing the technical component of the MRI must be accredited in MRI by the Intersocietal Accreditation Commission (IAC) or a MRI module by the American College of Radiology (ACR) or RadSite. The professional component of the MRI will be reimbursed based upon the accreditation of the facility as the ACR, IAC or RadSite facility accreditations require that interpreting professional physicians also be accredited.
MRI of the Shoulder
A. Magnetic resonance imaging (MRI) of the shoulder is considered medically necessary for the following indications:
Rotator cuff tendon abnormalities: full-thickness, partial-thickness, and recurrent (postoperative) tears, tendinopathy, tendinitis, and cuff tear arthropathy
Disorders of the long head of the biceps brachii: full-thickness, partial-thickness, and recurrent (postoperative) tears, tendinopathy, tendinitis, subluxation, and dislocation
Conditions affecting the supraspinatus outlet: acromial shape, os acromiale, subacromial spurs, acromioclavicular joint disorders, coracoacromial ligament integrity, subacromial bursitis
Labral abnormalities: cysts, degeneration, and tears, including superior labrum anterior posterior (SLAP) lesions, Bankart lesions and their variants, and recurrent (postoperative) labral tears
Abnormalities of the rotator interval and biceps pulley
Muscle disorders affecting the shoulder girdle: atrophy, hypertrophy, denervation, masses, injuries
Glenohumeral chondral and osteochondral abnormalities: osteochondral fractures and osteochondritis dissecans, articular cartilage degeneration, fissures, fractures, flaps, and separations
Intra-articular bodies
Synovial-based disorders: synovitis, bursitis, metaplasia, and neoplasia
Marrow abnormalities: osteonecrosis, marrow edema syndromes, and stress fractures
Neoplasms, masses, and cysts of bone, joint, or soft tissue
Infections of bone, joint, or soft tissue
Congenital and developmental conditions including dysplasia and normal variants
Vascular conditions: entrapment, aneurysm, stenosis, and occlusion
Neurologic conditions: entrapment, compression, masses, and peripheral neuritis
B. Magnetic resonance imaging (MRI) of the shoulder may be considered medically necessary to further clarify and stage the following conditions diagnosed clinically and/or suggested by other imaging modalities:
Arthritides: inflammatory, infectious, neuropathic, degenerative, crystal-induced, post-traumatic
Frozen shoulder and adhesive capsulitis
Primary and secondary bone and soft tissue tumors
Fractures and dislocations
C. Magnetic resonance imaging (MRI) of the shoulder may be considered medically necessary to evaluate the following specific clinical scenarios:
Prolonged, refractory, or unexplained shoulder pain
Acute shoulder trauma
Impingement syndromes: subacromial, subcoracoid, internal
Glenohumeral instability: chronic, recurrent, subacute, and acute dislocation and subluxation
Shoulder symptoms in the overhead or throwing athlete
Mechanical shoulder symptoms: catching, locking, snapping, crepitus
Limited or painful range of motion
Swelling, enlargement, mass, or atrophy
Patients for whom diagnostic or therapeutic arthroscopy is planned
Patients with recurrent, residual, or new symptoms following shoulder surgery
MRI of the Elbow
A. Magnetic resonance imaging (MRI) of the elbow is considered medically necessary for the following indications:
Ligament disorders (ulnar collateral, lateral ulnar collateral, radial collateral, and annular ligaments): sprains, partial and complete tears
Disorders of the flexor and extensor tendon origins (epicondylitis): tendinopathy, partial and complete tears
Distal biceps tendon disease: tendinopathy, partial and complete tears
Distal triceps tendon disease: tendinopathy, partial and complete tears, snapping, subluxation
Muscle and myotendinous injuries
Occult fractures
Osteochondral lesions: osteochondral fractures and osteochondritis dissecans
Cartilage lesions: chondral fractures and flaps, chondromalacia, degenerative arthritis
Joint effusions and inflammatory or proliferative synovitis
Intra-articular bodies: chondral, osteochondral, osseous
Symptomatic plicae, synovial folds, and elbow menisci
Olecranon and bicipitoradial bursitis: septic, traumatic, crystal-induced, inflammatory
Marrow abnormalities: bone contusions, osteonecrosis, marrow edema syndromes, stress fractures
Peripheral nerve disorders: entrapment, compression, cubital tunnel syndrome, muscle denervation
Congenital and developmental abnormalities
Neoplasms of bone, joint, or soft tissue
Infections of bone, joint, or soft tissue
Abnormalities of the proximal forearm interosseous membrane and neuromuscular structures
B. Magnetic resonance imaging (MRI) of the elbow may be considered medically necessary to further clarify and stage the following conditions diagnosed clinically and/or suggested by other imaging modalities:
Arthritides: primary inflammatory and erosive, infectious, neuropathic, degenerative, crystal-induced, post-traumatic.
Primary and secondary bone and soft tissue tumors
Fractures and stress fractures
Soft tissue injuries associated with a known fracture
C. Magnetic resonance imaging (MRI) of the elbow may be considered medically necessary to evaluate the following specific clinical scenarios:
Prolonged, refractory, or unexplained elbow pain.
Sports injuries, especially in throwing athletes
Elbow instability: acute, recurrent, chronic
Painful elbow snapping or mechanical symptoms
Refractory tennis elbow
Limited or painful range of motion, or contracture
Unexplained elbow swelling, mass, or atrophy
Neuropathy whose cause is localized to the elbow
Patients for whom diagnostic or therapeutic arthroscopy or elbow surgery is planned
Patients with recurrent, residual, or new symptoms following elbow surgery.
MRI of the Wrist
A. Magnetic resonance imaging (MRI) of the wrist is considered medically necessary for the following indications:
Abnormalities of the triangular fibrocartilage complex: partial tears, complete tears, and degeneration
Abnormalities of the scapholunate and lunotriquetral interosseous ligaments: sprains, partial tears, and complete tears
Abnormalities of the dorsal and volar extrinsic wrist ligaments
Ulnocarpal impaction syndrome
Fractures of the distal radius, scaphoid, and other carpal bones with normal or equivocal radiographs
Soft tissue injuries associated with distal radius fractures
Complications of scaphoid fractures: displacement, nonunion, malunion, and osteonecrosis
Osteonecrosis of the carpal bones
Ganglion cysts
Carpal tunnel syndrome: primary, secondary, and recurrent
Abnormalities of peripheral nerves: Guyon canal syndrome, entrapment , hamartomas, and tumors.
Flexor and extensor tendon disorders: partial and complete tears, tendinopathy, and tenosynovitis
Osteochondral and articular cartilage lesion
Vascular abnormalities: arterial aneurysms and pseudoaneurysms, varices, hemangiomas, and vascular malformations
Congenital and developmental conditions: dysplasia and clarification of normal variants
B. Magnetic resonance imaging (MRI) of the wrist may be considered medically necessary to further clarify and stage the following conditions diagnosed clinically and/or suggested by other imaging modalities:
Neoplasms of bone, joint, tendon sheath, or soft tissue
Infections of bone, joint, or soft tissue
Rheumatoid arthritis, juvenile idiopathic arthritis, psoriatic arthritis, gout, and related diseases
C. Magnetic resonance imaging (MRI) of the wrist may be considered medically necessary to evaluate the following specific clinical scenarios:
Acute and chronic wrist instability
Dorsal or ulnar-sided wrist pain
Wrist symptoms in adolescent gymnasts and other athletes
Unexplained chronic wrist pain
Acute wrist trauma
Wrist malalignments
Limited or painful range of motion
Unexplained wrist swelling, mass, or atrophy
Planning for diagnostic or therapeutic arthroscopy
Recurrent, residual, or new symptoms following wrist surgery
MRI of the Hand
A. Magnetic resonance imaging (MRI) of the hand is considered medically necessary for the following indications:
Tendon tears, including flexor/extensor tendons and adductor/abductor tendons of the thumb
Ligament tears, traumatic and/or degenerative, including gamekeeper’s thumb
Annular pulley injuries of the fingers
Subungual tumors, including glomus tumors
B. Magnetic resonance imaging (MRI) of the hand may be considered medically necessary to further clarify and stage the following conditions diagnosed clinically and/or suggested by other imaging modalities:
Inflammatory arthritis and tenosynovitis, including rheumatoid arthritis, juvenile idiopathic arthritis, seronegative arthritis, and connective tissue disease
Radiographically occult traumatic or stress/insufficiency fractures
Stener lesion of the thumb
C. Magnetic resonance imaging (MRI) of the hand may be considered medically necessary to evaluate the following specific clinical scenarios:
Prolonged, refractory, or unexplained pain or disability
Overuse syndromes
Bone and/or joint deformity with or without suspected soft tissue abnormalities, acquired or congenital
Trigger finger
Preoperative planning
Surveillance after surgery or postoperative complications
Magnetic resonance imaging (MRI) of the upper extremity is considered not medically necessary for all other indications, including when performed for screening purposes in asymptomatic patients (absence of signs, symptoms, or disease).
State Health Plan (SHP) Members:For dates of service on or prior to 12/31/2015, this policy applies to MRI of the upper extremity when performed in the emergency room. This policy does not apply to MRI of the upper extremity performed in non-emergency room settings. MRI of the upper extremity performed in non-emergency room settings require pre-certification by the Plan’s Utilization Review Vendor.
Effective 01/01/2016, this policy applies to MRI of the upper extremity performed in all settings for State Health Plan Members.
The coverage guidelines outlined in the Medical Policy Manual should not be used in lieu of the Member's specific benefit plan language.
Reimbursement for MRI imaging of the same anatomical area is generally limited to one (1) MRI imaging within a 6-month period. MRI imaging in excess of one (1) within a 6-month period will be subject to medical necessity review. Documentation should include radiology reason for study, radiology comparison study-date and time, radiology comparison study observation, radiology impression, and radiology study recommendation.
Additional MRI imaging of the same anatomical area may be appropriate for the following, including, but not limited to: diagnosis, staging or follow-up of cancer, follow-up assessment during or after therapy for known metastases, follow-up of member who have had an operative, interventional or therapeutic procedure (e.g., surgery, embolization), reevaluation due to change in clinical status (e.g., deterioration), new or worsening clinical findings, (e.g., neurologic signs, symptoms), medical intervention which warrants reassessment, reevaluation for treatment planning, follow-up during and after completion of therapy or treatment to assess effectiveness, and evaluation after intervention or surgery.
Re-imaging or additional imaging due to poor contrast enhanced exam or technically limited exam is the responsibility of the imaging provider.
Medically Necessary is defined as those services, treatments, procedures, equipment, drugs, devices, items or supplies furnished by a covered Provider that are required to identify or treat a Member's illness, injury or Nervous/Mental Conditions, and which Company determines are covered under this Benefit Plan based on the criteria as follows in A through D:
A. consistent with the symptoms or diagnosis and treatment of the Member's condition, illness, or injury; and
B. appropriate with regard to standards of good medical practice; and
C. not solely for the convenience of the Member, his or her Provider; and
D. the most appropriate supply or level of care which can safely be provided to Member. When applied to the care of an Inpatient, it further means that services for the Member's medical symptoms or conditions require that the services cannot be safely provided to the Member as an Outpatient.
For the definition of Medically Necessary, “standards of good medical practice” means standards that are based on credible scientific evidence published in peer-reviewed medical literature generally recognized by the relevant medical community, and physician specialty society recommendations, and the views of medical practitioners practicing in relevant clinical areas and any other relevant factors. BCBSMS makes no payment for services, treatments, procedures, equipment, drugs, devices, items or supplies which are not documented to be Medically Necessary. The fact that a Physician or other Provider has prescribed, ordered, recommended, or approved a service or supply does not in itself, make it Medically Necessary.
Investigative is defined as the use of any treatment procedure, facility, equipment, drug, device, or supply not yet recognized as a generally accepted standard of good medical practice for the treatment of the condition being treated and; therefore, is not considered medically necessary. For the definition of Investigative, “generally accepted standards of medical practice” means standards that are based on credible scientific evidence published in peer-reviewed medical literature generally recognized by the relevant medical community, and physician specialty society recommendations, and the views of medical practitioners practicing in relevant clinical areas and any other relevant factors. In order for equipment, devices, drugs or supplies [i.e, technologies], to be considered not investigative, the technology must have final approval from the appropriate governmental bodies, and scientific evidence must permit conclusions concerning the effect of the technology on health outcomes, and the technology must improve the net health outcome, and the technology must be as beneficial as any established alternative and the improvement must be attainable outside the testing/investigational setting.
