Magnetic resonance imaging (MRI) is a multiplanar imaging method based on an interaction between radiofrequency (RF) electromagnetic fields and certain nuclei in the body (usually hydrogen nuclei) after the body has been placed in a strong magnetic field. MRI differentiates between normal and abnormal tissues, providing a sensitive examination to detect disease. This sensitivity is based on the high degree of inherent contrast due to variations in the magnetic relaxation properties of different tissues, both normal and diseased, and the dependence of the MRI signal on these tissue properties.
MRI is a radiology technique that uses magnetism, radio waves, and a computer to produce images of body structures. The MRI scanner is a tube surrounded by a giant circular magnet. The patient is placed on a moveable bed that is inserted into the magnet. The magnet creates a strong magnetic field that aligns the protons of hydrogen atoms, which are then exposed to a beam of radio waves. This spins the various protons of the body, and they produce a faint signal that is detected by the receiver portion of the MRI scanner. The receiver information is processed by a computer, and an image is produced. The image and resolution produced by MRI is quite detailed and can detect tiny changes of structures within the body. For some procedures, contrast agents, such as gadolinium, are used to increase the accuracy of the images.
MRI of the spine is a powerful tool for the evaluation, assessment of severity, and follow-up of diseases of the spine. Spine MRI should be performed only for a valid medical reason. While spinal MRI is one of the most sensitive diagnostic tests for detecting anatomic abnormalities of the spine and adjacent structures, findings may be misleading if not closely correlated with the clinical history, clinical examination, or physiologic tests.
Spine MRI has important attributes that make it valuable in assessing spinal disease. Alternative diagnostic imaging tests include radiography, computed tomography (CT), myelography, and CT myelography. Compared with these other modalities, MRI does not use ionizing radiation. This is particularly advantageous in the lumbar area where gonadal exposure may occur, and in the cervical spine to avoid radiation to the thyroid. Myelography requires an invasive procedure to introduce intrathecal contrast agents. Both the puncture and the contrast agent can produce side effects and rarely significant adverse reactions. MRI allows direct visualization of the spinal cord, nerve roots, and discs, while their location and morphology can only be inferred on plain radiography and less completely evaluated on myelography. Compared to CT, MRI provides better soft tissue contrast and the ability to directly image in the sagittal and coronal planes. It is also the only modality for evaluating the internal structure of the cord.

Provider Accreditation for MRI – Network Providers
All Network Providers billing the technical component of the MRI must be accredited in MRI by the Intersocietal Accreditation Commission (IAC) or a MRI module by the American College of Radiology (ACR) or RadSite. The professional component of the MRI will be reimbursed based upon the accreditation of the facility as the ACR, IAC or RadSite facility accreditations require that interpreting professional physicians also be accredited.
MRI of the Cervical Spine
Magnetic resonance imaging (MRI) of the cervical spine is considered medically necessary for the following indications:
For evaluation of known or suspected multiple sclerosis (MS) a. Evidence of MS on recent baseline Brain MRI b. Follow up to known MS with changing signs or symptoms c. Follow up to the initiation or change in medication for patient with known Multiple Sclerosis
For evaluation of new or worsening neurologic deficits a. With any of the following new neurological deficits: extremity weakness, asymmetric reflexes, parasthesias, pain, myelopathy
For evaluation of chronic or degenerative changes (e.g., osteoarthritis, degenerative disc disease) a. With changing or new onset of radiculopathy and failure of conservative treatment for at least six (6) weeks b. With an abnormal electromyography (EMG) or nerve conduction study c. With exacerbation of chronic neck pain or new extremity numbness or tingling and unresponsive to trial of conservative treatment, including physical therapy or physician supervised home exercise plan (HEP), for at least six (6) weeks, unless contraindicated or unable to perform d. With chronic neck pain, with no neurological deficit, a negative x-ray and no recent (within past 3 years) cervical Spine MRI
For evaluation of new onset of neck pain a. Severe acute pain with radiculopathy b. With progression or worsening of symptoms during the course of conservative treatment c. With an abnormal electromyography (EMG) or nerve conduction study d. With Moderate to Severe pain without radiculopathy and failure of conservative treatment, including physical therapy or physician supervised home exercise plan (HEP) for at least 6 weeks
For evaluation of trauma or acute injury within past 72 hours a. Presents with radiculopathy b. With progression or worsening of symptoms during the course of conservative treatment c. With an abnormal electromyography (EMG) or nerve conduction studyd. Evaluation of acute, subacute, or chronic effects of trauma when pain is present in the absence of or not explained by abnormalities on other imaging to include x-rays or CTe. Evaluation of acute ligamentous injury and evaluation for dynamic instability on clinical testing or documented by x-rays
For evaluation of known tumor, cancer, or evidence of metastasis a. Staging of known tumor b. For follow-up evaluation of patient undergoing active treatment c. Presents with new signs (e.g., laboratory and/or imaging findings) of new tumor or change in tumor d. Presents with radiculopathy e. With an abnormal electromyography (EMG) or nerve conduction f. With evidence of metastasis on bone scan or previous imaging study g. With no imaging/restaging within the past ten (10) months
For evaluation of suspected tumor a. Prior abnormal or indeterminate imaging that requires further clarification
For evaluation of known or suspected infection, abscess, or inflammatory disease a. Paraspinal abscess as evidenced by neck pain, or laboratory or x-ray findings b. Osteomyelitis as evidenced on physical exam or laboratory x-ray findings c. Meningitis as evidenced by positive physical exam findings or history d. Septic arthritis or discitis as evidenced by physical exam or laboratory x-ray findings
For evaluation of immune system suppression (e.g., HIV, chemotherapy, leukemia, lymphoma) a. Presents with neck pain as a symptom of documented clinical findings of immune system suppression
For preoperative evaluation [if surgery scheduled within the next thirty (30) days] a. Known tumor and meets one of the tumor guideline criteria above b. Known infection and meets one of the infection guideline criteria above c. Known radiculopathy and failure of conservative treatment for at least six (6) weeks and when ordered by a neurosurgeon, orthopedist or surgeon d. With an abnormal electromyography (EMG) or nerve conduction study e. For pre-surgical Scoliosis survey in infant or child
For follow-up evaluation for surgery or fracture occurring within the past six (6) months a. Changing radiculopathy and failure of conservative treatment for at least (6) six weeks b. MRI of the Thoracic Spine
Magnetic resonance imaging (MRI) of the thoracic spine is considered medically necessary for the following indications:
For evaluation of new or worsening neurologic deficits a. With any of the following new neurological deficits: lower extremity weakness, lower extremity asymmetric reflexesb. Lower extremity parasthesias or myelopathy suspected of thoracic cord origin
For evaluation of chronic or degenerative changes (e.g., osteoarthritis, degenerative disc disease) a. With acute onset of point tenderness of a localized area of the spine b. With an abnormal electromyogram (EMG) or nerve conduction study c. With exacerbation of chronic back pain or new extremity numbness or tingling and unresponsive to trial of conservative treatment d. With chronic thoracic spine back pain, with no neurological deficit, a negative x-ray and no recent (within past 3 years) thoracic spine MRI
For evaluation of new onset of back pain a. Severe acute pain with radiculopathy b. With progression or worsening of symptoms during the course of conservative treatment c. With an abnormal electromyography (EMG) or nerve conduction study d. With moderate to severe thoracic spine level pain without radiculopathy and failure of conservative treatment for 6 weeks.

MRI of the Lumbar Spine
Magnetic resonance imaging (MRI) of the lumbar spine is considered medically necessary for the following indications:
For evaluation of new or worsening neurologic deficits a. With any of the following new neurological deficits: lower extremity weakness, asymmetric reflexes, evidence of Cauda Equina Syndrome, bowel or bladder dysfunction, new foot drop, parasthesias, myelopathy
For evaluation of chronic or degenerative changes (e.g., osteoarthritis, degenerative disc disease) a. With changing or new onset of radiculopathy and failure of conservative treatment for at least six (6) weeks b. With an abnormal electromyography (EMG) or nerve conduction study c. With exacerbation of chronic back pain or new extremity numbness or tingling and unresponsive to trial of conservative treatment, including physical therapy or physician supervised home exercise program (HEP), for at least six (6) weeks unless contraindicated or unable to perform d. With chronic low back pain, with no neurological deficit, a negative x-ray and no recent (within past 3 years) lumbar spine MRI
For evaluation of new onset of back pain a. Severe acute pain with radiculopathy b. With progression or worsening of symptoms during the course of conservative treatment c. Ankylosing Spondylitis- For diagnosis when suspected as a cause of back or sacroiliac pain and completion of the following initial evaluation: a. History of back pain associated with morning stiffness b. Sedimentation rate and/or C-reactive protein c. HLA B27 d. Non-diagnostic or indeterminate x-ray
Magnetic resonance imaging (MRI) of the spine is considered not medically necessary for all other indications, including when performed for screening purposes in asymptomatic patients (absence of signs, symptoms, or disease).
State Health Plan (SHP) Members:For dates of service on or prior to 12/31/2015, this policy applies to MRI of the spine when performed in the emergency room. This policy does not apply to MRI of the spine performed in non-emergency room settings. MRI of the spine performed in non-emergency room settings require pre-certification by the Plan’s Utilization Review Vendor.
Effective 01/01/2016, this policy applies to MRI of the spine performed in all settings for State Health Plan Members.
The coverage guidelines outlined in the Medical Policy Manual should not be used in lieu of the Member's specific benefit plan language.
Reimbursement for MRI imaging of the same anatomical area is generally limited to one (1) MRI imaging within a 6-month period. MRI imaging in excess of one (1) within a 6-month period will be subject to medical necessity review. Documentation should include radiology reason for study, radiology comparison study-date and time, radiology comparison study observation, radiology impression, and radiology study recommendation.
Additional MRI imaging of the same anatomical area may be appropriate for the following, including, but not limited to: diagnosis, staging or follow-up of cancer, follow-up assessment during or after therapy for known metastases, follow-up of member who have had an operative, interventional or therapeutic procedure (e.g., surgery, embolization), reevaluation due to change in clinical status (e.g., deterioration), new or worsening clinical findings, (e.g., neurologic signs, symptoms), medical intervention which warrants reassessment, reevaluation for treatment planning, follow-up during and after completion of therapy or treatment to assess effectiveness, and evaluation after intervention or surgery.
Re-imaging or additional imaging due to poor contrast enhanced exam or technically limited exam is the responsibility of the imaging provider.
Medically Necessary is defined as those services, treatments, procedures, equipment, drugs, devices, items or supplies furnished by a covered Provider that are required to identify or treat a Member's illness, injury or Mental Health Disorders, and which Company determines are covered under this Benefit Plan based on the criteria as follows in A through D:
A. consistent with the symptoms or diagnosis and treatment of the Member's condition, illness, or injury; and
B. appropriate with regard to standards of good medical practice; and
C. not solely for the convenience of the Member, his or her Provider; and
D. the most appropriate supply or level of care which can safely be provided to Member. When applied to the care of an Inpatient, it further means that services for the Member's medical symptoms or conditions require that the services cannot be safely provided to the Member as an Outpatient.
For the definition of medical necessity, “standards of good medical practice” means standards that are based on credible scientific evidence published in peer-reviewed medical literature generally recognized by the relevant medical community, and physician specialty society recommendations, and the views of medical practitioners practicing in relevant clinical areas and any other relevant factors. BCBSMS makes no payment for services, treatments, procedures, equipment, drugs, devices, items or supplies which are not documented to be Medically Necessary. The fact that a Physician or other Provider has prescribed, ordered, recommended, or approved a service or supply does not in itself, make it Medically Necessary.
Investigative is defined as the use of any treatment procedure, facility, equipment, drug, device, or supply not yet recognized as a generally accepted standard of good medical practice for the treatment of the condition being treated and; therefore, is not considered medically necessary. For the definition of Investigative, “generally accepted standards of medical practice” means standards that are based on credible scientific evidence published in peer-reviewed medical literature generally recognized by the relevant medical community, and physician specialty society recommendations, and the views of medical practitioners practicing in relevant clinical areas and any other relevant factors. In order for equipment, devices, drugs or supplies [i.e, technologies], to be considered not investigative, the technology must have final approval from the appropriate governmental bodies, and scientific evidence must permit conclusions concerning the effect of the technology on health outcomes, and the technology must improve the net health outcome, and the technology must be as beneficial as any established alternative and the improvement must be attainable outside the testing/investigational setting.
