Computerized (or computed) tomography, and often formerly referred to as computerized axial tomography (CAT) scan, is a radiologic procedure that combines many images with the aid of a computer to generate cross-sectional views and, if needed, three-dimensional images of the internal organs and structures of the body. Computerized tomography is more commonly known by its abbreviated names, CT scan or CAT scan. A CT scan is used to define normal and abnormal structures in the body and/or assist in procedures by helping to accurately guide the placement of instruments or treatments.
A large donut-shaped machine or scanner takes images at many different angles around the body. These images are processed by a computer to produce cross-sectional pictures of the body. In each of these pictures the body is seen as a "slice" of the body, which is recorded on a film. This recorded image is called a tomogram. "Computerized axial tomography" refers to the recorded tomogram "sections" at different levels of the body.
A CT provides clinical information in the detection, differentiation, and demarcation of disease. It is the primary diagnostic modality for a variety of presenting problems and is widely accepted as a supplement to other imaging techniques. CT examinations should be performed only for a valid medical reason and with the minimum exposure that provides the image quality necessary for adequate diagnostic information.
CT angiography (CTA) is a CT examination that is primarily performed for assessment of the heart, arteries, or veins of the body. It requires at a minimum a thin section helical (spiral) CT acquisition coupled with a power injection of intravenous iodinated contrast medium. Three dimensional rendering and multiplanar reformations are important components of many CTA examinations.

Provider Accreditation for CT – Network Providers
All Network Providers billing the technical component of the CT must be accredited in CT by the Intersocietal Accreditation Commission (IAC) or a CT module by the American College of Radiology (ACR) or RadSite. The professional component of the CT will be reimbursed based upon the accreditation of the facility as the ACR, IAC or RadSite facility accreditations require that interpreting professional physicians also be accredited.
I. Computed Tomography (CAT Scan)
A. Computed tomography (CAT scan) of the head or brain is considered medically necessary for the following indications:
Stroke
Transient ischemia attack (TIA)
Suspected subarachnoid hemorrhage (SAH)
Headache
Seizure
Central nervous system (CNS) infection or abscess
Follow-up evaluation of AVM
Hydrocephalus
Trauma
Decreased level of consciousness
Altered mental status
Tumor of primary CNS origin
Dementia
Suspected neonatal hemorrhage
Palpable mass of the skull or scalp
Follow-up evaluation of intracranial abscess
Follow-up evaluation of intracranial infection
Follow-up evaluation of primary brain tumor
Central nervous system evaluation for brain metastases
Follow-up evaluation of post-intracranial procedure, e.g. craniotomy, craniectomy
Follow-up evaluation post CNS shunt placement
Suspected CNS involvement with systemic disease, including but not limited to:a. Systemic lupus erythematosus b. Vasculitis c. HIV d. Sarcoidosis
Macrocephaly
Microcephaly
Craniosynostosis
Suspected subdural hematoma (SDH)
Arnold Chiari Malformation
Dandy Walker Cyst
Fibrous Dysplasia
Encephalocele
B. Computed tomography (CAT scan) of the maxillofacial area or neck is considered medically necessary for the following indications:
Complicated acute sinusitis with progression of symptoms even with medical management
Recurrent acute sinusitis (3 or more episodes within 1 year)
Chronic sinusitis (rhinosinusitis) (symptoms lasting 8 weeks or longer of varying intensity)
Suspected sinus or nasopharyngeal tumor
Submandibular or salivary gland mass or pathology
Mucocele or nasal polyp
Head or neck cancer
Trauma
Suspected submandibular duct stone or other salivary gland pathology
Cough, work up of chronic (cough lasting more than 3 weeks)
Parotid mass
Suspected head and neck abscess
Neck mass after equivocal ultrasound
Airway obstruction by neck mass
Suspected parathyroid tumor or pathology
Thyroid nodule, goiter or mass
Mucocele or nasal polyp
Suspected recurrent medullary thyroid carcinoma
Suspected submandibular duct stone
Suspected laryngeal fracture
Facial, orbital or head trauma
Suspected sinus malignancy
Evaluation of vocal cord paralysis or hoarseness
Lymphoma
Lymphadenopathy
Foreign body evaluation
C. Computed tomography (CAT scan) of the temporal bone, orbit, sella or posterior fossa, outer, or middle ear is considered medically necessary for the following indications:
Head or neck cancer
Suspected orbital tumor or other pathology
Suspected unilateral proptosis, orbital, periorbital mass or tumor
Unusual presentation of Bell’s Palsy
Suspected temporal bone, mastoid or ear disease, including but not limited to:a. Mastoiditisb. Cholesteatoma c. Tinnitusd. Conductive hearing loss e. Nystagmus f. Acoustic neuroma g. Sensorineural hearing loss h. Cerebellar pontine angle tumor
Trauma
Extraocular myopathy
Optic neuritis or suspected optic neuritis
Unusual presentation of Bell’s Palsy
Suspected temporal bone, mastoid or ear disease including but not limited to evaluation of tinnitus
Evaluation of vertigo
Evaluation of congenital anomaly of the ear
Evaluation of severe infections of the ear
Cochlear implant evaluation
Congenital hearing loss
Visual field loss
Congenital anomaly of the orbit
Otosclerosis
Suspected pituitary disease
Suspected cerebrospinal fluid leak with or without intrathecal contrast injection
D. Computed tomography (CAT scan) of the head and neck is considered not medically necessary for all other indications, including when performed for screening purposes in asymptomatic patients (absence of signs, symptoms, or disease).
II. Computed Tomographic Angiography (CTA)
A. Computed tomographic angiography (CTA) for pre-operative study for carotid endarterectomy or angioplasty is considered medically necessary for the following indications:
Symptomatic and asymptomatic carotid stenosis
Suspected anterior circulation ischemia
B. Computed tomographic angiography (CTA) of the head and neck is considered medically necessary for the following indications:
Cavernous hemangioma
Arterial and venous aneurysm
Subarachnoid hemorrhage (SAH)
Traumatic injuries to arteries and veins
Arterial dissection and intramural hematoma
Arterial and venous thromboembolism
Non-atherosclerotic, noninflammatory vasculopathy
Congenital vascular anomalies
Anatomic variants
Vascular interventions (percutaneous and surgical)
Vasculitis and collagen vascular diseases
Vascular infection
Vertebro-basilar insufficiency
Known or suspected parathyroid adenoma
Carotid body tumor
Known or suspected dural venous sinus thrombosis
Aneurysm follow up with or without prior treatment
Preoperative evaluation of neck mass
TIA by history with clinical changes
Pre-treatment planning of acute stroke intervention
Pre-treatment planning for parathyroidectomy
C. Computed tomographic angiography (CTA) of the head and neck is considered not medically necessary for all other indications, including when performed for screening purposes in asymptomatic patients (absence of signs, symptoms, or disease).
State Health Plan (SHP) Members: For dates of service on or prior to 12/31/2015, this policy applies to CAT scans of the head and CTA of the head when performed in the emergency room and non-emergency room settings. This policy applies to CAT scans of the neck and CTA of the neck when performed in the emergency room. This policy does not apply to CAT scans of the neck and CTA of the neck performed in non-emergency room settings. CAT scans of the neck and CTA of the neck performed in non-emergency room settings require pre-certification by the Plan’s Utilization Review Vendor.
Effective 01/01/2016, this policy applies to CAT scans and CTA of the Head and Neck performed in all settings for State Health Plan Members.
The coverage guidelines outlined in the Medical Policy Manual should not be used in lieu of the Member's specific benefit plan language.
Medically Necessary is defined as those services, treatments, procedures, equipment, drugs, devices, items or supplies furnished by a covered Provider that are required to identify or treat a Member's illness, injury or Mental Health Disorders, and which Company determines are covered under this Benefit Plan based on the criteria as follows in A through D:
A. consistent with the symptoms or diagnosis and treatment of the Member's condition, illness, or injury; and
B. appropriate with regard to standards of good medical practice; and
C. not solely for the convenience of the Member, his or her Provider; and
D. the most appropriate supply or level of care which can safely be provided to Member. When applied to the care of an Inpatient, it further means that services for the Member's medical symptoms or conditions require that the services cannot be safely provided to the Member as an Outpatient.
For the definition of medical necessity, “standards of good medical practice” means standards that are based on credible scientific evidence published in peer-reviewed medical literature generally recognized by the relevant medical community, and physician specialty society recommendations, and the views of medical practitioners practicing in relevant clinical areas and any other relevant factors. BCBSMS makes no payment for services, treatments, procedures, equipment, drugs, devices, items or supplies which are not documented to be Medically Necessary. The fact that a Physician or other Provider has prescribed, ordered, recommended, or approved a service or supply does not in itself, make it Medically Necessary.
Investigative is defined as the use of any treatment procedure, facility, equipment, drug, device, or supply not yet recognized as a generally accepted standard of good medical practice for the treatment of the condition being treated and; therefore, is not considered medically necessary. For the definition of Investigative, “generally accepted standards of medical practice” means standards that are based on credible scientific evidence published in peer-reviewed medical literature generally recognized by the relevant medical community, and physician specialty society recommendations, and the views of medical practitioners practicing in relevant clinical areas and any other relevant factors. In order for equipment, devices, drugs or supplies [i.e, technologies], to be considered not investigative, the technology must have final approval from the appropriate governmental bodies, and scientific evidence must permit conclusions concerning the effect of the technology on health outcomes, and the technology must improve the net health outcome, and the technology must be as beneficial as any established alternative and the improvement must be attainable outside the testing/investigational setting.
