Computerized (or computed) tomography, and often formerly referred to as computerized axial tomography (CAT) scan, is a radiologic procedure that combines many images with the aid of a computer to generate cross-sectional views and, if needed, three-dimensional images of the internal organs and structures of the body. Computerized tomography is more commonly known by its abbreviated names, CT scan or CAT scan. A CT scan is used to define normal and abnormal structures in the body and/or assist in procedures by helping to accurately guide the placement of instruments or treatments.
A large donut-shaped machine or scanner takes images at many different angles around the body. These images are processed by a computer to produce cross-sectional pictures of the body. In each of these pictures the body is seen as a "slice" of the body, which is recorded on a film. This recorded image is called a tomogram. "Computerized axial tomography" refers to the recorded tomogram "sections" at different levels of the body.
A CT provides clinical information in the detection, differentiation, and demarcation of disease. It is the primary diagnostic modality for a variety of presenting problems and is widely accepted as a supplement to other imaging techniques. CT examinations should be performed only for a valid medical reason and with the minimum exposure that provides the image quality necessary for adequate diagnostic information.
CT angiography (CTA) is a CT examination that is primarily performed for assessment of the heart, arteries, or veins of the body. It requires at a minimum a thin section helical (spiral) CT acquisition coupled with a power injection of intravenous iodinated contrast medium. Three dimensional rendering and multiplanar reformations are important components of many CTA examinations.

Provider Accreditation for CT – Network Providers
All Network Providers billing the technical component of the CT must be accredited in CT by the Intersocietal Accreditation Commission (IAC) or a CT module by the American College of Radiology (ACR). The professional component of the CT will be reimbursed based upon the accreditation of the facility as the ACR and the IAC facility accreditations require that interpreting professional physicians also be accredited.
I. Computed Tomography (CAT Scan)
A. Computed tomography (CAT scan) of the chest and thorax is considered medically necessary for the following indications:
Evaluation of cough
Evaluation of hemoptysis
Evaluation of vocal cord paralysis or hoarseness
Evaluation of unexplained wheezing
Evaluation of abnormal findings by prior chest imaging, including pulmonary nodule
Evaluation of lung cancer detected by bronchoscopy, cytology or other imaging
Suspected pulmonary embolism based on clinical assessment and pre-test probability testing (See Policy Guidelines)
Evaluation for metastases to lung or mediastinum, or restaging after completion of therapy
Known primary or metastatic lung or mediastinal tumor
Syndrome of Inappropriate Anti-diuretic Hormone
Suspected or evaluation of esophageal cancer
Interstitial lung disease
Suspected thoracic aortic dissection
Thoracic or thoracoabdominal aneurysm
Evaluation of chest trauma
Preoperative study for pneumothorax repair, pleural effusions or prior to Video Assisted Thoroscopic Surgery (VATS)
Evaluation of possible thymoma in patients with myasthenia gravis
Suspected bronchiectasis
Cystic fibrosis
Paraneoplastic syndrome suspicious for lung cancer
Fever of unknown origin
Evaluation of pneumonia
Evaluation of mediastinal mass
Evaluation of hilar adenopathy, prominent hilum, or elevated diaphragm identified by chest x-ray
Evaluation of new pleural effusion
Evaluation of lung abscess
Evaluation of known or suspected congenital or acquired disorder of the trachea

B. Computed tomography (CAT scan) of the chest and thorax is considered not medically necessary for all other indications, including when performed for screening purposes in asymptomatic patients (absence of signs, symptoms, or disease) except for indications outlined in the Screening for Lung Cancer Using Computed Tomography (CT) Scanning medical policy.
II. Computed Tomographic Angiography (CTA)
A. Computed tomographic angiography (CTA) of the chest and thorax is considered medically necessary for the following indications:
For evaluation of suspected pulmonary embolism based on clinical assessment and pre-test probability testing (See Policy Guidelines) (CT with contrast or CT pulmonary arteriography are both appropriate)
Developmental anomalies of the thoracic vasculature for initial evaluation, treatment planning and post-operative evaluation
Suspected thoracic aortic dissection (CT and MRI are alternative studies)
Aortic pathology
Assess thoracic venous structures
Peripheral arterial vascular disease
Aneurysm of the thoracic aorta or thoracoabdominal aneurysm
Trauma
Evaluation of anatomy of brachial arteries in the presence of hemoptysis
B. Computed tomographic angiography of the chest and thorax is considered not medically necessary when performed for screening purposes in asymptomatic patients (absence of signs, symptoms, or disease)
State Health Plan (SHP) Members:For dates of service on or prior to 12/31/2015, this policy applies to CAT scans and CTA of the chest and thorax when performed in the emergency room. This policy does not apply to CAT scans and CTA of the chest and thorax performed in non-emergency room settings. CAT scans and CTA of the chest and thorax performed in non-emergency room settings require pre-certification by the Plan’s Utilization Review Vendor.
Effective 01/01/2016, this policy applies to CAT scans and CTA of the chest and thorax performed in all settings for State Health Plan Members.
The coverage guidelines outlined in the Medical Policy Manual should not be used in lieu of the Member's specific benefit plan language.
Clinical assessment and pretest probability testing using the modified Wells criteria are required to support the use of a CT pulmonary angiogram to diagnose suspected pulmonary embolism. Patients with a high likelihood of pulmonary embolism include those with a modified Wells score of > 4. Patients with a low likelihood of pulmonary embolism include those with a modified Wells score ≤ 4.

Medically Necessary is defined as those services, treatments, procedures, equipment, drugs, devices, items or supplies furnished by a covered Provider that are required to identify or treat a Member's illness, injury or Mental Health Disorders, and which Company determines are covered under this Benefit Plan based on the criteria as follows in A through D:
A. consistent with the symptoms or diagnosis and treatment of the Member's condition, illness, or injury; and
B. appropriate with regard to standards of good medical practice; and
C. not solely for the convenience of the Member, his or her Provider; and
D. the most appropriate supply or level of care which can safely be provided to Member. When applied to the care of an Inpatient, it further means that services for the Member's medical symptoms or conditions require that the services cannot be safely provided to the Member as an Outpatient.
For the definition of medical necessity, “standards of good medical practice” means standards that are based on credible scientific evidence published in peer-reviewed medical literature generally recognized by the relevant medical community, and physician specialty society recommendations, and the views of medical practitioners practicing in relevant clinical areas and any other relevant factors. BCBSMS makes no payment for services, treatments, procedures, equipment, drugs, devices, items or supplies which are not documented to be Medically Necessary. The fact that a Physician or other Provider has prescribed, ordered, recommended, or approved a service or supply does not in itself, make it Medically Necessary.
Investigative is defined as the use of any treatment procedure, facility, equipment, drug, device, or supply not yet recognized as a generally accepted standard of good medical practice for the treatment of the condition being treated and; therefore, is not considered medically necessary. For the definition of Investigative, “generally accepted standards of medical practice” means standards that are based on credible scientific evidence published in peer-reviewed medical literature generally recognized by the relevant medical community, and physician specialty society recommendations, and the views of medical practitioners practicing in relevant clinical areas and any other relevant factors. In order for equipment, devices, drugs or supplies [i.e, technologies], to be considered not investigative, the technology must have final approval from the appropriate governmental bodies, and scientific evidence must permit conclusions concerning the effect of the technology on health outcomes, and the technology must improve the net health outcome, and the technology must be as beneficial as any established alternative and the improvement must be attainable outside the testing/investigational setting.
