About Coding Policy

Generally, BCBSMS follows CPT, HCPCS and ICD-10-CM national coding guidelines. Coding has a direct affect on reimbursement. It can affect the timeliness and accuracy of claim payments. The use of Coding Policy is not intended to replace independent medical judgement for treatment of individuals.

  1. Coding Policies available at this site represent only a small portion of the coding policies and procedures used in the editing and processing of claims. BCBSMS plans to make more Coding Policies available on this site. You should also be aware that Coding Policies are updated periodically and are, therefore, subject to change. We will make reasonable efforts to update the policies on this site, but you should be aware that some of the Coding Policies displayed may be out of date.
  2. Policies and procedures included in the Coding Policy are not intended to certify coverage availability. While policy may exist on this website for a particular service, procedure, equipment, supply, drug, etc ., it may be excluded in a member's benefit plan. Please check with the Customer Service Department to determine if the service in question is a covered benefit under a particular benefit plan. In the event of a conflict between Coding Policy and any Plan document under which a member is entitled to Covered Services, the Plan document will govern. Plan documents include, but are not limited to, Subscriber Contracts, Summary Plan Documents, and other coverage documents prepared by a Plan.
  3. Coding Policy is subject to Health Insurance Portability and Accountability Act (HIPAA) regulations.
  4. Coding Policies are the property of BCBSMS and you are strictly prohibited from using them for any commercial uses whatsoever. Commercial use does not include use of the Coding Policies for purposes related to the health care of a BCBSMS Plan member. In addition, CPT codes and descriptions are the properties of the American Medical Association with all rights reserved.The AMA assumes no liability for data contained or not contained on this website.
