DESCRIPTIONOracea is indicated for the treatment of only inflammatory lesions (papules and pustules) of rosacea in adult patients. No meaningful effect was demonstrated for generalized erythema (redness) of rosacea. Oracea has not been evaluated for the treatment of the erythematous, telangiectatic, or ocular components of rosacea.
Previous use of samples or vouchers/coupons will not be considered for authorization.
Oracea will be approved when ALL of the following are met:
If the brand name medication is approved, the Plan will only reimburse the same amount that is reimbursed for generic doxycycline monohydrate capsules immediate release (2 capsules per day) prescriptions.
POLICY EXCEPTIONSOracea prior authorization is not required for Federal Employee Program (FEP) and State Health Plan members.
POLICY GUIDELINESInvestigative service is defined as the use of any treatment procedure, facility, equipment, drug, device, or supply not yet recognized by certifying boards and/or approving or licensing agencies or published peer review criteria as standard, effective medical practice for the treatment of the condition being treated and as such therefore is not considered medically necessary.
The coverage guidelines outlined in the Medical Policy Manual should not be used in lieu of the Member's specific benefit plan language.
