DESCRIPTIONDifferin (adapalene) is a modulator of cellular differentiation, keratinization, and inflammatory processes, all of which represent important features in the pathology of acne vulgaris. It may normalize the differentiation of follicular epithelial cells resulting in decreased microcomedone formation.
POLICYDifferin 0.3% Gel will be approved when ALL of the following are met:
**If approved the brand name medication is approved the Plan will only reimburse the same amount that is reimbursed for generic adapalene 0.1% gel prescriptions.
POLICY EXCEPTIONSDifferin 0.3% gel prior authorization is not required for Federal Employee Program (FEP) and State Health Plan members.
POLICY GUIDELINESInvestigative service is defined as the use of any treatment procedure, facility, equipment, drug, device, or supply not yet recognized by certifying boards and/or approving or licensing agencies or published peer review criteria as standard, effective medical practice for the treatment of the condition being treated and as such therefore is not considered medically necessary.
The coverage guidelines outlined in the Medical Policy Manual should not be used in lieu of the Member's specific benefit plan language.
