The "be smoke-free" Program provides a comprehensive 90-day tobacco-cessation program that includes telephone counseling, visits with a "be smoke-free" Network Provider and medication or nicotine replacement therapy. The program is provided at no cost -- and if you are successful in being tobacco-free for 12 months, and your household is tobacco-free, your Blue Care premium will be reduced.

Blue Care Limitations and Exclusions
The Cost Awareness policy is available to individuals transferring from another Blue Cross and Blue Shield plan. This $100,000 lifetime maximum policy includes benefits for hospital care, maternity care, outpatient services, psychiatric care, and substance abuse.
